Job Summary

The Community Health Worker, under general direction, will conduct outreach and educational visits to participants of community-wide campaigns relating to promotion of healthy lifestyles, including physical activity and nutritional strategizing. This position is focused on prevention interventions, nutritional and physical activity guidance, and motivational support. The Community Health Worker is responsible for engaging community members at-risk, such as minorities, low income communities, and individuals with low educational attainment, by providing educational material, outreach messages, and collecting data pertaining to programs. Temporary grant-funded position.

Key Functions/Knowledge/Skills

• Higher level of accuracy expected; may train new employees; ability to make critical decisions.
• Conduct assessment of health promotion programs that involve community health workers at the local/regional level with at-risk populations.
• Engage stakeholders, identify current capacity and needed resources determine timelines and document implementation plans.
• Establish rapport with community members to aid in behavior change.
• Create partnerships with city and school entities in the community.
• Encourage community members to participate in community-based project activities and education.
• Provide small and/or large group educational classes on physical activity, healthy eating, disease prevention, and breastfeeding.
• Assist in data collection efforts including surveys and focus groups. This includes tracking participants’ activities in programs and making referrals for unmet care and education needs.
• Communicate participants’ questions and/or concerns to project leadership.
• Assist in identifying health promotion programs in the area that involve community health workers.
• Conduct contact tracing with members of the community.
• Ensure that the data set passes data test and provides results to establish baseline participation and degree of involvement.
• Discuss project materials and messages with community members and other members of the program.
• Proficient in working with the public and key stakeholders.
• Must know a variety of strategies to engage and encourage behavioral change.
• Most know how to explain scientific information and data to the public.
• Be able to establish effective and harmonious work relationships with the company staff.
• Must know how to create clear project reports.
• Must be able to effectively carry out assignments to achieve the goals of the program.
• Perform other duties as assigned.

Minimum Education & Experience

Bachelor's degree (no experience), or associate's degree plus two years of experience, or high school graduation or GED plus four years of experience.

Special Certifications & Licenses

Valid Texas Driver’s License.

Independence & Judgement

Average judgment; must recognize and resolve problems such as bad grammar, type of procedure to apply, where to refer call or correspondence without specified destination, when to change or sharpen blade on mower, whether there is a problem/malfunction with a piece of equipment that needs attention, etc.; may involve freedom in selection of work methods among approved routines, setting of priorities, or sequencing of tasks within established schedules.

Initiative & Ingenuity

General guidelines: requires a lot of interpretation and non-standard responses to varied situations; have to figure out where to go for answers (not all sitting there in procedures manual); must take multiple factors into account; some control over priorities.

Responsible for use of equipment/property such as office equipment, supplies, etc., which are used in office.
